Form Title: | Claim Form for Reimbursement of Paternity Leave (Applicable to Staff Already Ceased Employment Only) |
Brief | For a staff paid out of Salaries Grant but with his employment ceased, if he has covered his absence by no-pay leave/annual leave due to failure of providing documentary proof for taking the paternity leave within the designated period, he can provide documentary proof to the school within 12 months after the first day on which he has taken the paternity leave or within 6 months after the cessation of employment (whichever period expires first). The school should pay him the paternity leave pay not later than 7 days after the document is provided. The school may claim reimbursement of the salaries paid to the staff by completing this claim form. |
